Board Letter

As your Board is aware, the Sheriff’s Office has an independent contractor agreement for health care services at the County’s correctional services. As the original contract is expiring, the Sheriff’s Office recently issued a Request for Proposal (RFP). This letter provides background information and requests that your Board approve the agreement with California Forensic Medical Group (CFMG) in the amount of $3,645,624 for medical and dental services at the County’s Correctional Facilities.

 

Background

 

Since 2012, the County has contracted with an outside provider for comprehensive medical, dental, pharmaceutical services and cooperative mental health services for inmates of the County’s Correctional facilities. The contractor is responsible for the provision of health care services to inmates from the time an inmate is committed to the custody of the jail until release of the inmate from custody. The program includes inmate medical screening, admissions evaluation, triage, sick call, care of medically fragile inmates, pharmacy services, psychiatric screening, suicide risk assessments, and the continuing care of identified health problems, detoxification and emergency services.

 

A RFP was prepared and issued, and proposals were received from vendors. Due to the complex set of services required and significant changes regarding contract requirements and licensing, the due diligence required for the selection process was considerable. The proposals and interviews were conducted by a broad committee comprised of the County Sheriff's Office, Health Services, and an outside physician. A consensus of the interview team was that the highest-rated vendor was CFMG. The notice of intent to award was issued on May 31, 2016, and final negotiations with the vendor commenced immediately. Those negotiations led to the contract documents proposed in this Board report.

 

As you are aware, the County is required to provide to all inmates constitutionally-adequate medical care that is consistent with the quality of care available in the community. All health care services will comply with Title 15 and 24 of the California Code of Regulations and will meet the Institute for Medical Quality (IMQ) Accreditation Standards for the Jail. CFMG will work collaboratively with County Mental Health and the Sheriff's Office to ensure compliance with IMQ standards and to coordinate IMQ accreditation process to seek accreditation by September 30, 2017. The funding for the contract is contained in the Sheriff's Office FY 2016-17 budget.

 

As part of this RFP process, the awarded vendor will expand services to the new Rountree facility once that project has been completed. It is the goal of the Sheriff’s Office to reduce recidivism by housing nearly all AB109 inmates at Rountree in order to provide valuable programming, treatment and education to long term inmates while they are in custody. The Corrections Bureau has concluded that providing medical services at Rountree is a top priority. The contract also includes more dental procedures to be completed on site. Mental Health services will continue to be provided by the Health Services Agency and the Crisis Intervention Team staff. CFMG will meet with Mental Health services on a daily basis.

 

Summary and Recommendations

 

CFMG has the required expertise to manage costs using Utilization Management (UM) technologies to reduce unnecessary and costly referrals to outside care, the buying power to negotiate bulk purchasing rates for medical supplies, pharmaceuticals and contracted specialty care, and a large enough employment base to implement efficient staffing patterns.

 

It is, therefore, RECOMMENDED that your Board approve the agreement with CFMG in the amount of $3,645,624 for medical and dental services at the County’s Correctional Facilities, and authorize the Sheriff-Coroner to sign the agreement on behalf of the County.
