Recommended Action(s):

1.              Approve a revenue agreement with Janus of Santa Cruz, Contract #18R0187 in the amount of $666,076, to provide limited term staffing from the Health Services Agency (HSA) Clinic Services Division for provision of Medication-Assisted Treatment, and authorize the HSA Director to sign the grant agreement.

 

2.              Adopt a resolution to accept and appropriate unanticipated revenues from the revenue agreement in the amount of $111,013 for fiscal year 2017-18.

 

3.              Approve a staffing addition to the HSA budget of six full-time equivalent (FTE) limited-term positions for two years within the HSA Clinics Services Division to expand existing Medication-Assisted Treatment programs, and direct HSA to refer these positions to the Personnel Department for classification, recruitment, and hiring.

 

Executive Summary

The Health Services Agency (HSA) requests approval of a revenue agreement (attached) in the amount of $666,076 from Janus of Santa Cruz (Janus). The contract establishes HSA as a sub-grantee for grant funding awarded to Janus in the total amount of $4.6 million from the California Department of Healthcare Services (DHCS) in partnership with the federal Substance Abuse and Mental Health Services Administration (SAMHSA). The $666,076 sub-award funds are being provided to HSA for a two-year period, allowing HSA to add staffing of six FTE limited-term positions for two years within HSA Clinics Services Division to expand existing Medication-Assisted Treatment (MAT) programs. 

 

Background

In July 2017, Janus was awarded the ‘MAT Hub & Spoke’ grant from DHCS/SAMHSA. For this grant, only narcotic treatment programs (NTP) were allowed to apply, and Janus is the only NTP in Santa Cruz County that applied. In August, 2017, HSA was invited to become a sub-awardee for this grant, wherein Janus, as an NTP, is the ‘hub’, and HSA, providing clinic services, is the ‘spoke’.

 

Analysis

The ‘hub & spoke’ paradigm is an evidence-based model that assigns an NTP as the ‘hub’, and clinic services as a ‘spoke' when coordinating various levels of care for the treatment of substance use disorders. Janus and HSA agreed on the attached sub-contract whereby Janus provides HSA $666,076 for two years of staffing. The grant money will be applied to staffing positions to expand MAT services. These positions would provide intensive case management and nursing support to patients in HSA’s MAT program, expand outreach services in the community, and support HSA’s clinics with supervising medical case managers to manage this complex patient population. Hiring these positions will allow HSA’s Clinic Services Division to increase the number of MAT patients seen, resulting in increased revenue and program sustainability. Funding is available for each of these positions for a 24 month period from the date of hire. The grant allows Janus to extend the grant term beyond December 31, 2019 to provide for a full two-years of service for these positions, based on HSA’s ability and timing to hire. HSA anticipates the approximate two-year timeline to span the fiscal years of 2018-19 through 2019-20. HSA projects that all six positions will be successfully hired by no later than December 2018, resulting in a 24 month term ending no later than December 31, 2020.
